Paul, Just updated to the 2018 version. Doing test prints on the tax forms, the W-3 has the following errors:
Block 15 data is print in Block 16
Block 16 and 17 data is being printed in "Employer's contact person."
Employer's Phone # is printed in "Employer's e-mail address"
Employer's contact person is printed in "Employer's Fax #"
Employer's Fax # is printed over-top of "Under penalties ----"
Employer's e-mail address is not printed.
In other words, Everything from Block 15 on down is printed in the next lower line.

Hi Allen, Not sure where you got your forms from but they are not the computer forms we support. I just did a test print and then scanned it into PDF and will send it to you via email so you can see how everything lines up as expected.

They are the official IRS forms requested from the US Government.

You just got lucky as in my experience since 1993, it is very rare when the government forms were the same as computer forms.

OOOOps, My error, I was printing EVERYTHING one block low. Was just looking at the bottom entries, didn't pay attention to the whole printout.
Sorry for causing the confusion.
