944 2nd page: name & EIN zero?

When I run the 944 form on Payroll 14.0.4, the employer name and EIN show up fine on page 1, but incorrectly at the top of page 2, appearing as zeroes. There is some garbage in 13a as well.

PayWindow is date sensitive so you need to set the date to 13/31/2015.
