I ordered W2 forms from formsfulfilment. I have the W2s in a pdf file, but when I print to the forms, nothing lines up with the predetermined boxes. Is that because it is from a pdf? Should I print direct from zpay to the W2 form?

Yes a PDF is not the same size as actually printing tothe printer. See the instructions at this link, in particular the link to thehelp file on that page:
