I set up our Simple plan deduction with a check in the fit box and a check in the pension box and d 12a for w-2. It is including it in Box 1 of the W-2 and it shouldn't but I need it included in Box 3 &5. What do I need to do differently?

See this page linked below to the help file on setting up the Deductions. You did not check the 941 Box (see the description for Item 7) And if it is required in one of the boxes like 10 through 14, see that information too.
