941 Balance dueand Pay Stub

I'm having trouble with showing the balance due on the 941. In the set up window, I've marked 2500 or less due and no payment made. However, on the 941 line 12d has the amount filled in that is actually balance due (which should be on line 14.

Also, on the Payment Voucher -- I check 4th quarter and all 1, 2nd, 3rd circles fill in.

We assume that everyone paid the taxes before printing the 941 since we do not track tax payments. The 941 Form is an Adobe Acrobat for that allows you to edit and change anything you desire on it.
