Simple IRA Deduction

My program all of a sudden will not calculate employee and employer contributions and subtract them from the gross pay ( I have 2% marked in the appropriate box). How do I correct this?

My guess is that the employee met the cut off amount that you set for the deduction. Read the descriptions for items 3, 4 and 5 in this page of the on-line copy of the help file:
