disappearing info

So, from time to time I have to edit vacation and sick accruals. I did so today and after editing info, the record disappeared for this date. I don't get it.

Then, I had to look back on a previous transaction. I clicked on the transaction and instead of hitting <cancel> I hit <save> and the record disappeared. What gives?

I just edited a few entries and had no problem. Is it possible that you click the "Minus" button for Delete by accident since it is next to the Edit button?

Save and Close in that Window deletes the transaction.
