Utah tax table

I've updated to the latest tax tables and looking at the Utah tax table there is a comment "see tax notes for Utah" but I can't find tax notes. Where do I see the tax notes for Utah?

The Utah tax notes were shown during the installation on the page that said "READ THIS FIRST". Here is the info for Utah:

UT - Utah changed their tax formula in 2008 that would not fit into our setup for tax tables. So the formula was coded into the PayWindow 6.0.11 and newer. You will still need to attach the Utah tax record to each employee so the program knows the employee is to be taxed in Utah.

Utah state requires that you set the Withholding allowance credit in the tax table edit tool. Open Utah for editing and select the tab labeled "State II". In the Exemption Amount filed we have the default for a weekly payroll which is 2.00. Use this table for your payroll cycle:
Weekly: 2.00
Biweekly: 5.00
Semimonthly: 5.00
Monthly: 10.00
Quarterly: 31.00
Semiannual: 63.00
Daily: 0.00
