Where do I enter Line 6 of the Employees W-4

One of my employees filled out #6 on the W-4 form which states that she would like an additional amount withheld from her paycheck for taxes in the amount of $3.

I need to know where to enter this amount and how can I specify that this is additional tax withholdings?

In the employee setup, the bottom right corner where is asks for "additional state tax" to be withheld.
